I've tried every combination possible both inside Premiere and at the OS level. I don't think this is a preferences issue - if it was we'd have found it by now. Just to give a bit more information about my setup, my Blackmagic audio out feeds into an analogue mixer (for panning and volume control) which is then connected to the studio speakers. Yes it fixes the issue, but it introduces the problem of having to use the Mac's own terrible speakers to monitor my audio. I don't want to play audio through the Mac's built-in speakers, which is what happens if you set audio playback to built-in output. Playback preferences control where it plays back, My Blackmagic card is fairly old now so might not be officially supported by Adobe or Blackmagic. I've cleaned out my media cache and that doesn't help.

It happens in all projects on my machine, but not on another Mac (2013 Mac Pro, same OS but with an Blackmagic Mini Monitor) I'm using the latest drivers, although this issue has been happening for at least a year with various different drivers. This happens only if my video and audio playback is set to my Blackmagic card. Video playback will continue without any issues, but the audio will drop out as will the audio meters inside Premiere. anything done to an audio clip will mean the audio cuts out.
